A waiver is required for all participants before entering The Kids Clubhouse. Click on the WAIVER box below to sign the waiver for you and your child(ren).  If you are an adult signing a waiver for yourself only, you can sign under "Just Me."  If you would like a physical copy of the waiver, please see front desk attendant after submitting the electronic waiver.

To make the admission process fast and easy, sign the WAIVER online before getting to The Kids Clubhouse location.  A signed waiver is valid for every consecutive visit after the 1st visit, until notified otherwise by the adult participant.
